Dear Patients,

This is to formally notify you regarding the permanent closure of the neurology practice of Hector F. Colon, MD, PA, located at 2700 S. Clear Creek Road Killeen, Texas. Dr. Colon retired from the practice of medicine and closed the physical office location on April 1, 2026.

Our office manager remained available by telephone through September 30, 2026, to assist with medication refills and medical record transfers and to ensure continuity of care and support your transition to other care providers.

A formal long-term plan has been established to secure and preserve all patient medical records.

Please note the following critical updates regarding how to access or transfer your medical records:

Discontinued Phone Number: The practice’s previous telephone number (254-516-2343) has been permanently disconnected since September 30, 2026.
Designated Records Custodian: Sertus Medical Billing has officially assumed the role of legal custodian of records for the practice.

New Contact Information: To request copies of your medical records or to have them transferred directly to a new healthcare provider, you, your legal representative or your new physician must contact Sertus Medical Billing.
Dedicated phone numbers: (855) 732-5116 and (512) 575-8028 as an alternate number.

Yes it is true. Dr Colon has retired, effective immediately. I will be in and out of the office if you need anything, leave a message. Medication’s will be covered up to 90 days. Contact your pharmacy. however narcotics and controlled substance will not. You will have to contact your primary care for that and a new referral to another neurologist. We are working diligently to find a replacement, but we all know Dr. Colon could never be replaced.
