LifeTime Resources is excited to provide SFC services for individuals enrolled with United Healthcare and Humana.
👨‍👩‍👧‍👦 SFC supports caregivers who live with the person they care for—helping loved ones stay safely at home, not in a facility.
Caregivers receive:
✔️ A daily stipend
✔️ Ongoing support from a care coach
✔️ Help navigating services
📌 Requirements:
• Live in the same home
• Enrolled in a Medicaid Waiver (Pathways)
• Background check (no violent offenses)

What can you do to stop nursing home fraud?
📑 Read your Medicare Summary Notice (MSN) or Explanation of Benefits (EOB) to compare the services Medicare was charged with what you received and to determine medical necessity for these services.
🎁Do not accept gifts (such as money, gift cards, or groceries) in return for choosing a nursing home in which to receive skilled nursing.
🖊Do not sign forms that you do not understand, including a Medicare Outpatient Observation Notice (MOON), an Advance Beneficiary Notice (ABN), or any form that is blank.
Contact the to report nursing home care fraud.

Did you know you can go online and check if there have been any claims made against your Medicare number that shouldn't be there? Here is how you can check for suspicious durable medical equipment claims.

1. Log in or create an account at Medicare.gov.
2. From the Home screen, click on “Check my claims.”
3. Change the Date Range to “Last 36 months,” and click ""Apply.""
4. Change the Claim Type to “Durable medical equipment claims,” and click ""Apply.""
5. Review claims for anything not received or not requested.

If you find something concerning, report it to the SMP at smpresource.org/reportfraud.
