Ontario Health Association of Family Health Teams of Ontario (AFHTO) Jane Philpott

Patient attachment is only the beginning.

At the AFHTO 2026 Summit, Carefirst Family Health Team facilitated an interactive workshop “Access Before Attachment – Co-Designing Culturally Responsive Pathways Into a Neighbourhood Health Home.”

We shared our experience as a community-sponsored FHT, where trusted community partners are not an outreach add-on but part of the access infrastructure, alongside the Carefirst Access Clinic and team-based care that keeps people connected after attachment

A full room of peer FHTs, community support services partners, and academic colleagues added their wisdom on outreach, system navigation, and standards of care in primary care. Together, we generated a shared vision, practical strategies, and immediate next steps.

No single organization can make a neighbourhood health home real alone. Thank you to AFHTO and to everyone who joined the dialogue.

🌟 Looking Back: Medical Imaging for the Golden Ages

Back in March, we were delighted to welcome Dr. Weldon Liu, Radiologist, who shared his knowledge and expertise in a fascinating 90-minute session, “Medical Imaging for the Golden Ages.”

With over 100 participants joining us, it was a wonderful opportunity for our community to gain a better understanding of medical imaging and its role in healthcare.

It’s hard to believe that six months have passed since we last met Dr. Liu! Many community members have been looking forward to welcoming him back for more insightful sharing. Are you one of them? 😊

🎶 A Heartfelt Thank You to Voices United Charity Corp! ❤️

Carefirst extends our heartfelt gratitude to Voices United Charity Corp for organizing the Voice United Charity Singing Campaign, which began in November 2025 and culminated in a finale concert on September 19, 2026, in support of Carefirst’s Campus of Care!

Through months of dedication, music, and community spirit, Voices United brought people together for a wonderful cause. We are deeply grateful to the Campaign Organizing Committee, the charity performers, sponsors, donors, volunteers, and supporters who contributed to the campaign’s success.

Thanks to this incredible collective effort, $55,000 was donated to Carefirst to support the development of our Campus of Care!

This generous contribution will help us bring our vision of integrated, community-based care to life, creating a welcoming place where seniors and community members can access the care and support they need to live their best lives as they age.

Together, we are building more than a campus — we are building a caring community where everyone can thrive. 💖

A meaningful day of learning, connection, and collaboration at “From On Lok to Ontario: how 50 years of evidence-based lessons learned from PACE (Program of All-Inclusive Care for the Elderly) can advance community-based primary health care transformation closer to home”, a Symposium hosted at Toronto Metropolitan University.

Carefirst was honoured to participate in this important conversation, alongside leaders from On Lok PACE, Ontario Health, TMU, UofT and community care. We are proud to be part of this and remain committed to advancing integrated, equitable, and person-centred care closer to home.

What a meaningful day for Carefirst Seniors as we excitedly came together for our Annual General Meeting, 50th Anniversary Celebration & Recognition Ceremony — marking five decades of compassion, partnership, innovation, and commitment to our community.

🏆 A special highlight was the Carefirst 50th Anniversary Breakthrough Awards, recognizing individuals, organizations, and community partners whose vision, leadership, generosity, and dedication helped Carefirst achieve significant milestones, breakthrough barriers, and transform the delivery of community health and senior care.
