Our DPH Home Visiting program supports moms and babies during pregnancy and after birth – from pregnancy through the baby’s first year – a time when guidance, care, and connection can make a real difference.

The program is also designed to help families early, before minor issues become serious challenges or complications.

Families do not need to be in crisis to qualify. Any high-risk pregnant woman or infant is eligible.
