When "Manuel” first came to Community Care Clinic, his diabetes was dangerously uncontrolled with an A1C over 15.5. He was weak, frail, and struggling just to stand up from a chair.

Today, after medication, diabetes education, physical therapy, and compassionate support from our care team, Manuel’s A1C is down to 7.8 — and he’s getting stronger every day.

At a recent follow-up appointment, he and his wife both cried tears of relief and gratitude when they heard the news.

This is what compassionate healthcare can do.

Because of supporters like you, patients in our community like Manuel are finding hope, healing, and a path toward healthier futures. ❤️

On April 30, the women of St. Paul's Episcopal Church hosted a Women’s Wellness Event where they put together 100 beautiful bags of feminine care supplies for the patients of the CCC!
