Social Worker (BSW)

The Pigeon Falls Health Care Center has an opening for a (BSW) Certified Social Worker. Hours will be mostly daytime. Work in a family atmosphere that provides safety for our residents and staff.

Pay Range $27.83 – 29.72

Required Duties
• Provides social work assistance to residents with a diagnosis of or often with co-occurring mental illnesses, intellectual disabilities, substance abuse or aging needs
• Collaborate with the Psychiatrist and the interdisciplinary treatment team in providing recovery focused person-centered care to residents
• Prepare care plans, MDS and other reports as needed. Document pertinent information in social services progress notes. Secure necessary authorizations to release information.
• Works with resident, guardian, county, family, legal representatives, and advocates on care/recovery plans and discharge/future plans
• Coordinates and participates in progress meetings and care conferences with residents, county agencies, guardians, family, advocates and other interdisciplinary treatment team members
• Provide crisis intervention and emergency response services as needed
• May conduct investigations regarding alleged abuse or misconduct, resident grievances, and other concerns as directed

• Comply with Federal, State and County law and codes
• Follow employment policies and procedures established by Trempealeau
• Follow employment policies and procedures established by Trempealeau County Health Care Center
• Be knowledgeable of and adhere to resident's rights
• Other duties as deemed necessary or assigned

Education Requirements
• Minimum of a bachelor’s in social work or bachelor’s in human services and eligible for Social Work Training Certificate

What a beautiful night we had on Thursday, July 23rd as we hosted our annual Brownie and Ice Cream Social here at Pigeon Falls Health Care Center. Wally Tranberg and Friends were here with live entertainment, pulled pork dinners was served by our Citizen Advisory Board. Thank you to our staff and volunteers who put baskets together for the raffle. A special thank you to everyone who joined and supported our event. We hope to see you all again next year!
