Our Hospital Transition Program walks alongside individuals as they return home, helping them stay safe, connected, and on the path to healing. From basic needs to ongoing support, we’re there so no one has to navigate recovery alone.

Together for West Virginia. Real stories. Real impact. Real love in action.💙💛

On a hot August day, an elderly neighbor came to our Parkersburg office with his electric about to be shut off. Not long after, the power was gone, leaving him in the middle of a heat wave without relief or rest.

Our team stayed with him through it, reaching out, checking in, and working with local partners. Thanks to their support and Sisters Health Foundation, his power was restored. He shared how grateful he was to finally go home, cool off, and sleep again.

No one should have to face moments like this alone.
