🎉Big news from Chester County: chronic homelessness has been made rare and brief.
This milestone reflects years of work by cross the community, particularly among dedicated partners who regularly participate in Case Conferencing meetings - led by Open Hearth Inc., including:
• Chester County Government departments (Chester County Department of Community Development, Adult Probation, Parole, & Pretrial, Children, Youth & Families, Drug & Alcohol, Human Services – Human Needs Network, Mental Health/Intellectual & Developmental Disabilities, Office of Aging, Chester County Prison)
• Emergency shelter providers ( Ann's Heart of Phoenixville Code Blue and CERAH Program, Community, Youth, & Women's Alliance, Inc, Friends Association, Good Samaritan Services, Safe Harbor of Chester County, St. Mary's Franciscan Shelter for Homeless Families, WC Atkinson)
• and a wide network of case management, housing, financial, and treatment providers (ACTnow, Brandywine Valley Active Aging, Chester County Intermediate Unit, Coatesville VA Medical Center, Elwyn, Haven Behavioral Hospital of West Chester, Holcomb Behavioral Health, Home of the Sparrow - Pennsylvania, Human Services, Inc., KACS, North Star of Chester County, Oxford SILO)
Together, we built a system that knows every person experiencing chronic homelessness by name, connects them to housing and supports, and tracks outcomes in real time.
Today, Chester County marks the functional zero milestone for chronic homelessness. Progress is possible. This work is proof.
