Last week, NTSB Chairwoman Jennifer Homendy joined Sen. Dan Sullivan, Alaska Department of Transportation and Public Facilities Commissioner Ryan Anderson, and FAA Regional Director Erik Amend in Anchorage for the Alaska Aviation Safety Roundtable.

During the discussion, the Chairwoman emphasized the importance of implementing NTSB safety recommendations to help prevent future accidents and improve safety for the communities that rely on aviation every day.

In Alaska, aviation is an essential connection to people, goods, and services. Collaboration among government and industry partners is critical to addressing the state's unique aviation challenges and advancing solutions that strengthen aviation safety.

A missing lock ring in a critical flight control system led to the loss of pitch control and the 2022 crash of a DHC-3 Otter seaplane, killing the pilot and all 9 passengers aboard.

Following the investigation, NTSB safety recommendations prompted De Havilland Canada to update maintenance procedures, improve inspection guidance, encourage the removal of unapproved components, and clarify assembly instructions.

Today we remember aviation pioneer Wally Funk, whose remarkable career helped open doors for generations of women in aviation, aerospace, and transportation safety.

A trailblazer throughout her life, Funk became one of the first women to complete the Mercury 13 astronaut training program, proving she had the skill, determination, and resilience. She never gave up on her dream to become an astronaut, finally reaching space in 2021 at the age of 82.

Beyond her achievements in aviation and space exploration, Funk made history as the first female investigator at the National Transportation Safety Board and the first female flight inspector for the Federal Aviation Administration. Throughout her career, she logged more than 19,600 flight hours, taught thousands of pilots, and dedicated her life to advancing aviation safety.

Wally Funk's perseverance, passion for flight, and commitment to safety continue to inspire aviators, investigators, and future generations who dare to break barriers.

We extend our condolences to her family, friends, and all who were inspired by her extraordinary life. Fly high, Wally.

NTSB Recommendations Address Hidden Underground Pipeline Risks

Natural gas pipeline incidents remain a serious risk nationwide, especially when underground hazards are not identified, evaluated, or managed.

After the fatal 2023 natural gas explosion in West Reading, Pa., federal pipeline safety regulators and the organization that develops 811 “Call Before You Dig” best practices acted on NTSB safety recommendations to help operators identify hidden hazards, evaluate heat-related risks, and improve 811 participation.
