Congratulations NAIPC-Baltimore for earning a 2026 Make More Happen™ Award from Liberty Mutual Insurance and Safeco Insurance! The funding will help the Baltimore Chapter expand its programs beyond the 19 senior centers and four libraries it currently serves, while increasing outreach to homebound older adults and caregivers – ensuring more individuals have access to critical education and resources.

US Home & Auto Insurance Group partnered with NAIPC-Baltimore to support its mission of helping older adults safely and successfully age in place. Agency principal Patrick Geoghan was introduced to the organization through his wife, Mary Lynch, the chapter’s chair, and quickly recognized the need for accessible insurance education around in-home care, home modifications, and financial protection.

“NAIPC offers a trusted platform to educate and protect vulnerable populations,” said Geoghan. “We’re proud to support their work and help provide practical resources that allow seniors to maintain independence and give families peace of mind.”

Ryann is a government affairs and public policy strategist with more than a decade of experience working at the intersection of healthcare, aging, disability policy, and federal decision-making. She has led advocacy and policy initiatives across Congress and federal agencies, helping mission-driven organizations advance priorities with clarity, credibility, and long-term impact.

NAIPC proudly recognizes member Jangir Sultan, Founder & CEO of Patient Advocates of America, on being named a 2026 Health Care Trailblazer, as reported by City & State New York.

Sultan was honored for his leadership in advancing aging-in-place initiatives, care coordination, and improving healthcare transitions for families across New York. His work supports older adults and medically complex individuals in remaining safely in their homes while navigating healthcare, insurance, and long-term care systems.

A former licensed nursing home administrator and licensed occupational therapist, Sultan brings experience across acute care, rehabilitation, and community-based settings. Through Patient Advocates of America, he provides independent care management focused on safety, dignity, and continuity of care.

Consuela Marshall, OT, CHAT, CAPS, CDP, CGCP is a licensed occupational therapist with nearly 30 years of experience in home health and rehabilitation. She is a Certified Home Accessibility Therapist, Certified Dementia Practitioner, and Certified Aging-in-Place Specialist. As a longtime family caregiver, she founded the Finding Foothold Foundation to make safety education more equitable, proactive, and accessible—especially in underserved communities. She is also the author of Fall Prevention: Planning Not to Fall.

Restoring Independence at Home:
Designing Regional Systems for Sustainable, Replicable Aging-in-Place Models

Healthcare systems, housing providers, and aging services organizations face growing pressure to move beyond isolated home modifications. Coordinated, scalable aging-in-place strategies are increasingly being implemented in efforts to meet the needs of the aging population.

This session explores how clinical assessment, environmental risk governance, and cross-sector system design can transform fragmented referrals into integrated regional operating models supporting sustainable aging at home.

Attendees will learn to identify high-risk environmental zones and align functional mobility needs with institutional design and accessibility standards. Participants will also gain guidance on supporting implementation decisions through coordinated clinical, housing, and community leadership that advances aging-in-place initiatives.

The session will provide a practical governance framework for improving outcomes, reducing system strain, and strengthening partnerships across healthcare, housing, and aging services ecosystems. This includes those that serve older adults with complex clinical and environmental needs.

Speaker Bio

Dr. Comfort Uwadiae, DPT, CEAS, ECHM, CAPS is the Founder and Managing Member of Opt2Restore LLC, a New York based aging-in-place, home modification consulting and governance firm. She is the creator of the Opt2Restore™ C.A.R.E. Model, a standards-based framework aligning healthcare delivery with housing systems and community partners to support safer care transitions, environmental risk reduction, and long-term independence.

A former physical therapy provider and Clinical Instructor at Montefiore Hospital, Dr. Uwadiae brings over 13 years of experience across acute critical care, inpatient rehabilitation, outpatient services, and home-based care. She maintains active clinical engagement through Catholic Health, supporting transitions from healthcare systems into real-world living environments.

Her work focuses on building scalable, accountable aging-in-place and cross-sector integration systems for aging populations and medically complex patients. She collaborates with healthcare organizations, housing partners, and community coalitions to develop governance structures and implementation pathways for regional aging-in-place initiatives.
