What does meaningful, lasting change actually look like?

At NYFS, our Transformational Model of Care guides how we work alongside individuals and families every day. It is a holistic, relational approach to care that centers connection, lived experience, and the strengths each person brings.

Our model is rooted in the diverse services we offer and reflects the “NYFS way”, care that is trauma-informed, culturally responsive, and grounded in dignity and connection. It recognizes that meaningful change happens over time, through relationships that support healing and growth.

This approach shapes how we listen, respond, and support individuals and families in ways that are thoughtful, consistent, and responsive to their unique needs. At its core, this is how we support people in ways that feel meaningful and lasting.

According to The Trevor Project’s 2025 U.S. National Survey on the Mental Health of LGBTQ+ Young People, "44% of LGBTQ+ young people who wanted mental health care in the past year were not able to get it".

The survey captures insights from more than 16,000 LGBTQ+ young people ages 13 to 24 across the United States. Behind these numbers are real experiences shaped by stigma, rejection, and limited access to safe, supportive spaces.

Creating environments where individuals feel seen, supported, and safe can make a meaningful difference in well-being and is central to the work we do every day at NYFS.
