Shelby County Community Services, Community Day Services program has an opening for a Case Manager aka QIDP. This is a full time position M-F 8-430. Qualifications include a four year degree and experience working with individuals with disabilities. Check out SCCS CDS page to see some of the things we do.
QIDP duties include plan writing for individuals, maintaining files for individuals as well as assisting in some of the daily activities. Benefits in addition to working with some really awesome people are paid holidays, retirement plan, paid sick, vacation, and personal days, health insurance , and optional vision and dental insurance.

Our services include: ◦ Respite Services, that striveto keep families together and engaged with their developmentally disabledloved one, while providing a dedicated team assisting individuals with dailyliving, socializing, and learning, and attending to their specific healthcare needs. ◦ Activity Center Program,dedicated to providing a positive supportive environment to help those whoare seriously and chronically mentally disabled maintain and learn new strategiesfor managing their mental illness, learning community life skills, interactingsocially with others, and facilitating as far as possible integration intothe community. ◦ Developmental TrainingProgram, provides skills training for adults with developmental andmental disabilities resulting in serious functional skills deficits withthe skills, allowing each individual to maximize their potential and functionas independently as possible in their daily lives. ◦ Residential Services, offeringCommunity Integrated Living Arrangements and Supported Living Arrangementsdesigned to meet the specialized diagnosed needs of our clients. These housingoptions allow the most independent environment possible, while providingfamilies and clients the assurance that support staff are available to meetthe physical, emotional, and health care needs of those that live under ourcare. ◦ Manufacturing / ProductionFacilities, a primary goal of which is to provide vocational skillstraining and employment opportunities for individuals with developmentaldisabilities or other applicable obstacles to employment. Our productionfacilities manufacture paper plates, plastic film, trash bag liners, andfood-grade plastic bags. We also provide warehousing and distribution servicesfor these and related products for the state of Illinois. Our Greenhouseproduces various ornamental flowers and vegetables throughout the year, andspecializes in growing pot mums and asters for autumn, and poinsettias forChristmas. ◦ Outpatient Mental HealthClinic, to provide professional psychiatric, psychological, andcounseling services to individuals, couples, families and groups, as wellas consultation and outreach education to other local agencies, groups, andprofessional and community organizations. ◦ DUI and Substance AbuseServices, dedicated to helping persons who are experiencing problemswith drug and alcohol addictions, whether with co-occurring mental illnessor not. Individuals are offered counseling, education and recovery treatmentoptions, socialization and skills training, and potential employment in ourproduction unit.
