During National EMS Week, we’re recognizing the paramedics and EMTs who show up on people’s worst days, ready to help at a moment’s notice.

You can help them help you.

The San Marcos Patient Information Form gives first responders quick access to critical information like:

• Medications
• Allergies
• Medical history
• Emergency contacts

In an emergency, you may not be able to communicate clearly. Having this form completed and easy to find can save valuable time and help EMS provide faster, safer care.

We encourage everyone to:

✔ Download the form
✔ Fill it out completely
✔ Keep it updated
✔ Place it on your refrigerator or save it on your phone

First responders are trained to check the refrigerator for emergency medical information.

This simple step can make a big difference for you, your family, and the emergency crews responding to help.
