The major goal for Extra Mile Social Services is to assist vulnerable adults that are incapacitated due to their physical or mental health remain safely in their own homes. Also, a major goal for Extra Mile Social Services is to assist individuals that use emergency rooms and medical expresses for their medical needs rather than using their Primary Care Physician. Extra Mile Social Services will
address the needs of individuals that 18 years or older and assist them with following their medical treatment plan. The ultimate goal for those individuals that participate in the services that Extra Mile Social Services is to keep the individual from being referred to Adult Protective Services because of a simple lack of support in the home. Case Coordination Advance Medical Directives
Personal Care Prescription Pick Up
Light Housekeeping Errands
Transportation Bill Payment
Lawn Care Pet Care
Follow Up Doctor Appointment (Scheduled and Transportation Provided)
Professional Services Available:
Registered Nurse
Licensed Practical Nurse
Certified Nursing Assistance
Certified Home Health Aide
Certified Dialysis Technician
Medical Social Worker
Prices are $22.00 per hour (Sliding Scale Available)
Services are provided in 3 to 4 hour blocks of time that can be divided on an as needed basis.
