The Wayne County Place Matters Team is one of sixteen Place Matters Teams located throughout the country, working in partnership with the National Collaborative of Health Equity to improve the health of communities by addressing the Social Determinants of Health as a root cause that leads to poor health. Each Place Matters team is focused on an issue specific to their community. Here in Wayne Coun

ty our focus is on Infant Mortality, in which the Infant Mortality rates among Black Americans are three times higher than
White Americans. Addressing how Place and Race play apart in health disparities will help us better understand how we can achieve health equity, but health equity can only occur when we understand the obstacles that have caused these inequities in the first place. Our mission is to strengthen the communities into which babies are born in by fostering cross-sector engagement and collaboration to improve opportunities for healthy living, education, and economic development in Wayne County. Place Matters hopes to achieve this through non-traditional UPSTREAM solutions which addresses the root cause of the problem.
