**Reminder for Parents/Guardians: Nonmedical Immunization Exemption**

If your child has a Certificate of Nonmedical Exemption on file, a new form must be completed each year.

* The current certificate expires **June 30**.
* A **new Certificate of Nonmedical Exemption** must be completed **before the first day of school**.
* Please ensure the updated form is **submitted to the school nurse** prior to the start of the school year.

Students must have a valid, current certificate on file to remain in compliance.

**Immunization Reminder**

If your child receives immunizations, please make sure they are **up to date on all required vaccines before the first day of school**. This includes any grade-specific requirements (such as Tdap for 6th grade). We encourage you to schedule appointments early to avoid the back-to-school rush. Updated immunization records must be **submitted to the school nurse** prior to the start of school to ensure your child can attend without interruption.
