UCP Connections

UCP Connections We provide person-centered planning, case management, fiscal intermediary services, and resource referral for adults with disabilities.

UCP Support Services Brokerages servin people who live in Multnomah and Clackamas counties. Brokerage customers receive state and federal funds to purchase the support they need to reach their individual life goals. Once referred to UCP Connections and/or UCP Mentors, our customers are connected with a Personal Agent to facilitate their access to support services. Brokerage customers can utilize

their brokerage services towards achieving their goals related to community inclusion, independent living, employment, adaptive equipment, select therapies, family support, behavioral interventions, and other areas of life. Our Services:

The principles of self-determination guide our program, which means that the individual directs their brokerage services, makes decisions about their life and supports, and chooses how to prioritize their goals. Freedom, authority, autonomy, responsibility, and confirmation are integral to how we provide services. Core Values:

Clackamas County issued an evacuation warning for the Clackamas River, Molalla River, Johnson Creek, and south of Welches area. Please be careful out there!

A Level 3 (Go Now!) evacuation notice has been issued for Shady Dell Road and Fawn Lane. There is extreme danger in your area and you should evacuate now. Leave immediately, it is unsafe for you to stay in the area. Your life could be in great danger. Do not stop to gather your belongings or protect your home. Follow your emergency plan. Leave as fast and as safely as possible.

🌧️ A strong atmospheric river will bring heavy rain and widespread river flooding across northwest Oregon and southwest Washington on Thursday. Some rivers may continue to flood through at least Saturday. Heavy rain may also bring landslides as well as flooding of urban and low-lying areas, creeks, and streams. If you live in a flood-prone area, have a plan ready!

In 2024, we received 28,932 requests for help with food and meals — up from 25,124 in 2023.

Food assistance is consistently one of the top three needs people contact us about. We can connect you with food resources in your area, including pantry and meal locations, assistance programs, and help accessing SNAP (OR) and Basic Food (SW WA).

If you're in need of a place to cool down during the extreme heat, cool down at one of the cooling centers across the county. For a full list of cooling centers, go to: www.clackamas.us/relief.

Also be sure to check on your loved ones (including your pets), neighbors, friends and those who may have medical conditions that may make them vulnerable to the heat.

We know that Medicaid saves lives, we know that cuts to Medicaid threaten the health and wellbeing of people with disabilities across the country. The Collaboration to Promote Self Determination put together a very comprehensive overview of what cuts will mean.
