Dear Central Middle School Athletes, and Families,
As we prepare for an exciting 2026-27 athletic year, Port Huron High School is transitioning to Final Forms for all athletic registrations, forms, and physical management. This platform will replace our previous system (Athletic Packets) to streamline the clearance process for our student-athletes.
Please review the critical updates below regarding mandatory athletic physicals, upcoming summer schedules, and tryout clearance requirements.
📋 Action Required: Final Forms Registration & Physicals
To participate in any tryouts, practices, or conditioning, all athletes must be completely cleared through Final Forms. No athlete will be allowed to step onto the field or court until this is complete.
Account Creation: Both parents/guardians and student-athletes must create an account on the Final Forms platform and digitally sign all required medical and district consent forms.
Physical Upload Requirement: You must upload a scanned copy or clear photo of both sides of the athlete's physical examination form directly to Final Forms.
Validity Date: To be valid for the entire 2026-27 school year, the physical examination must be dated after April 15, 2026.
Forms: Step-by-step instructions for navigating the new Final Forms platform are attached to this communication, along with a blank copy of the standard MHSAA Physical Form.
