Feeling tired, overwhelmed, or isolated because of your chronic disease? The Chronic Disease PATH program gives you the tools to feel better. The telephone workshop meets for 1 hour, once a week for 6 weeks. You must have a telephone to attend this workshop. When you register, you agree to attend all six sessions. Please let your facilitator know if you are going to be absent. This workshop is free for Michigan residents. Family members, friends, and caregivers are encouraged to attend with you.
