The Industrial Commission of Arizona (ICA) is proud to announce the official statewide launch of the modernized Form 108 (Recommended Average Monthly Wage) & Updated Form 104 on September 22, 2026! 🛠️✨
We've completely overhauled the legacy filing process by introducing a streamlined dual form. Form 108 Standard for baseline wages and Form 108 Supplemental for complex wage scenarios. We are increasing transparency, improving legal compliance, and drastically reducing calculation errors!
🔑 KEY ROLLOUT HIGHLIGHTS:
1️⃣ Statewide Outreach: The ICA is broadcasting updates to adjusters, carriers, self insured employers and, TPAs, and attorneys across all communication channels to ensure seamless ecosystem alignment. 2️⃣ Continuation of Webinar Training: Live training webinars are open and continuing throughout the upcoming weeks!
