All Elders and caregivers are welcome to join us at the Senior Center from 9–9:30 am for a relaxing 30-minute neighborhood stroll around Confederated Way at your own pace.

Join us on June 18 from 5–7 pm at the Yellowhawk Laxsimwit Conference Room and Basketball Court for dinner, games, activities, and giveaways for kids and dads. Program graduates will also be celebrated!

Before any appointment at Yellowhawk, please check in at either the Elder Entrance or the Main Entrance to get started.

All patients are required to complete annual updates—keeping your information current helps your visit go smoothly and ensures you receive the best care possible. You may be asked to review or update documents, so be sure to connect with an Eligibility Coordinator to confirm everything is up to date.

Yellowhawk patients can connect with a live nurse when the clinic is closed, thanks to our after-hours support line provided by AMBS.

This service is available evenings, weekends, and holidays—from 5 PM to 7 AM. Simply call 541.966.9830 to get the care and guidance you need, anytime.
