Autumn Years Foundation The Autumn Years Foundation is dedicated to making sure seniors are aware of the great services, resources and programs their community has to offer.

If you, or someone you know, is a senior in need of a little extra assistance then please reach out to us today. How much does this cost? Our service is supported through grants and donations so there is no charge for our service to you. If you feel we are very helpful and would like to donate or refer us to someone you feel would like to donate to our cause then please let your care coordinator k

now so we can take care of that with you. What is the process to get help? We will start your call by greeting you, identifying your chief complaint and then completing our intake which usually takes about 10-15 minutes. After intake is completed we will review your concerns with you, make sure we understand your goals and set a follow up call within 1-3 business days. After the call, your care coordinator will collaborate with specialists in our office to form a care plan and then note their recommendations in your file so that we can share them with you later.
