Our Story
Beacon of LIFE is an affiliate of AcuteCare Health System, whose mission is to provide the newest healthcare and social service choices for elders living in Monmouth County.
The Beacon of LIFE center was established to deliver PACE (Program of All-Inclusive Care for the Elderly) services. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.
Under PACE, participants must receive all needed health care, including primary care and specialist physician services (other than emergency services) from the PACE organization or from an entity authorized by the PACE organization. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE services. Emergency services are covered.
Participation requirements:
• You are 55 years of age or older.
• You live in the service area of a PACE organization.
• You are certified as meeting the need for the nursing home level of care in New Jersey.
• You are able to live safely in the community with the help of PACE services.
PACE services include but are not limited to the following:
• Primary Care
• Social Services
• Physical Therapy
• Occupational Therapy
• Nutritional Services / Meals
• Home Care Services
• Hospital Care
• Emergency Services
• Medical Specialty Services
• Prescription Drugs
• Nursing Home Care
• End of Life Care
• Dental Care
• Adult Day Care with Recreational Activities
• Laboratory / X-ray Services
• Transportation
PACE also includes other services determined necessary by your team of healthcare professionals to improve and maintain participants’ overall health.
Enrollment is voluntary and participants may disenroll at any time.
Beacon of LIFE accepts Medicaid, Medicare or private payment. If you are eligible and enroll in PACE and are Medicaid or Medicare and Medicaid eligible, you pay no fees for all authorized health care services. Participants may be fully liable for the costs of unauthorized or out-of-PACE program services.
An Enrollment Specialist will meet with you, your family member or caregiver, in your home to tell you about the PACE program. The Enrollment Specialist will discuss eligibility requirements for PACE and schedule a visit for you at the Beacon of LIFE center. You will have an opportunity to meet with the PACE Interdisciplinary team. If you are considering enrollment, you will receive a medical evaluation and be assessed by the PACE Interdisciplinary team so that a plan can be developed to provide the care you need, which will include input from you. The Enrollment Specialist will review the plan of care and the enrollment paperwork with you and anyone you designate to represent you. Enrollment is voluntary and you may disenroll at any time.
If you qualify for Medicare, all Medicare-covered services are paid for by PACE. If you also qualify for your State’s Medicaid program, you pay nothing for the long-term care portion of the PACE benefit. If you don’t qualify for Medicaid, you will be charged a monthly premium to cover the long-term care portion the PACE benefit. PACE also covers Medicare Part D drugs if you are eligible. In PACE, there is never a deductible or copayment for any drug, service or care approved by the PACE team.
