The Caregiver Program of SNAP

The Caregiver Program of SNAP Providing support for informal caregivers with services at home, in the community, and by phone. We serve Queens Community Districts 8–14.

Respite • Supplemental Services • Individual Counseling • Support Groups • Educational Workshops • Case Assistance Hours of operation have changed due to Covid-19. The office is open from 8:30am to 4:30pm Monday, Wednesday and Friday. We service Community Boards 8, 9, 10, 11, 12, 13 and 14. To join the program call us at 718-527-5380. We will do a simple intake over the phone and then a case worker will come out to meet you and your love one in the home. Due to Covid-19 assessments can be done over the phone. At that point you can take advantage of any or all of the services listed. We look forward to working with you and your loved one.

September is Frontotemporal Dementia (FTD) Awareness Month, a time to shine a light on a lesser-known form of dementia and the individuals and families it impacts.

While Alzheimer's disease often begins with memory loss, Frontotemporal Dementia (FTD) typically affects behavior, personality, decision-making, and language first. Because these changes can be mistaken for stress, depression, or other conditions, understanding the signs is an important step toward earlier diagnosis and support.

Awareness leads to understanding, and understanding helps reduce stigma. By learning more about FTD, we can better support those living with the disease and the caregivers who walk beside them every day.

💜 September is World Alzheimer's Month, a time to raise awareness, reduce stigma, and show support for the millions of individuals and families affected by Alzheimer's disease and other forms of dementia.

As former President Jimmy Carter said:

"We become not a melting pot but a beautiful mosaic. Different people, different beliefs, different yearnings, different hopes, different dreams."

This quote reminds us that every person has a unique story, experiences, and strengths that deserve to be honored. A dementia diagnosis does not define who someone is. By fostering understanding, compassion, and inclusion, we can help build communities where people living with dementia—and those who care for them—feel seen, valued, and supported.
