Bring it to your appointment. We can share it with you through email or you can pick up a printed copy at the school.

CareNow is a partner with BVSD in our Workers Comp treatment. They will be offering sports physicals for $35.00 for students starting May 1st, 2026, through August 31st, 2026. This will include the CHSAA forms. The nearest CareNow venue is in Westminster, just off Hwy 36 and W 92nd (5 miles south of Broomfield HS).
