Support for Families with Loved Ones in Long-Term Care
At Rockie Skye SADC, we know how important it is to ensure your loved one is receiving the quality of care they deserve—even when you’re not there in person. Our Nursing Home & Assisted Living Advocacy service is designed to provide consistent oversight, communication, and peace of mind.
This service includes:
• Routine Wellness Check-ins: Regular visits or calls to monitor your loved one’s physical and emotional well-being.
• Care Plan Oversight: We review care plans with facility staff to ensure services are being delivered as promised.
• Caregiver & Staff Communication: We maintain open communication with nursing home staff and alert families to any concerns or changes.
• Attendance at Care Plan Meetings: We attend scheduled care meetings on your behalf (in person or virtually) to advocate for your loved one’s needs.
• Facility Visit Reports: You receive a written summary after each visit with notes on condition, environment, and any follow-up needed.
• Problem Resolution: If issues arise (e.g., medication errors, changes in condition, poor hygiene), we advocate directly with facility leadership to resolve them quickly.
• Family Updates: Regular communication to keep family members informed, especially for out-of-town caregivers.
• Support with Transfers or Reassessment: If relocation or changes are needed, we help assess options and coordinate transitions.
