Child to Family Connections (CFC) is a fully licensed non-profit organization that provides services to foster connections and keeping families together.
Swan Services: Working with forever families who are open for adoption, this program completes child profiles, family profiles, child preparation services and post-permanency services for children in the area. Home and Community Habilitation: Giving adults with disabilities the opportunity to become active members of our community, this program pairs individuals with a support specialist to enhanc
e one's lifestyle. Foster Care: This program works with families who want to open their homes for children in need. We use a personal approach that allows us to connect with these families and offer support along the way. Transitional Housing: Working with homeless young adults, the transitional housing program assists individuals with stable housing while they work towards personal growth and independence. Family Group Decision Making: This program utilizes all family members and support by involving them in meetings where decision making takes place regarding children in protection or need. Adoptive Services: This program specializes in adoptive home studies and post-placement support services.

Child to Family Connections was incorporated in April 2002 as a private, nonprofit licensed foster care agency in Crawford County. Its purpose was to help foster families become licensed and receive placements. The agency has grown in numbers and programs. CFC now employs over 40 full and part-time employees. We provide many licensed and contracted services in Crawford, and Venango Counties. Services include Home and Community Services, Foster Care, State Wide Adoption Network (SWAN), Family Group Decision Making, Transitional Housing, and Family Behavior Therapy.

Adoption Services
CFC workers provide prospective adoptive parents with the resources and guidance necessary to complete an adoption.
Adoptive Home Studies
As a state-licensed adoption agency CFC staff compile and write adoptive home studies that meet state requirements for the finalization of both state (SWAN) and private adoptions.
Family Behavioral Therapy
Family Behavior Therapy, (FBT), is an in-home, evidence-based program aimed at reducing drug and alcohol use in adults and youth.

The 12-month program focuses on 6 main components: stable housing, educational training and growth, job and career exploration and training, physical health and well-being, as well as financial awareness.
