Under the direction of the Office of Social Services, the Homeless Engagement and Assessment Response Team seeks to connect with community partners and identify gaps in serving individuals experiencing homelessness.

Our team responds to 311 calls and proactively identifies potential hazards like unsafe locations, open flames, or debris. By addressing these risks early, we help create safer conditions for everyone.

This work isn’t just about cleanup — it’s about understanding needs, reducing harm, and connecting individuals to support when it’s needed.

We’re grateful to partner with Re:Center Ministries, an organization providing critical shelter and long-term support for individuals in our community.

From overnight shelter for men to safe spaces for women and children, their work meets immediate needs while also creating pathways toward long-term stability.

HEART’s outreach teams are often the first point of connection — building relationships in the field and helping bridge trust so individuals feel comfortable accessing services like those offered at Re:Center.

📞 Call 911 if there is an immediate emergency or a situation that threatens life or safety.

📞 Call 311 for non-emergency concerns related to homelessness, encampments, or situations where someone may need outreach or assistance from the HEART team.

311 helps connect concerns to the appropriate outreach and risk assessment teams so they can respond and follow up.

When in doubt, remember:
