• SOCIAL SECURITY CARD (An adult 18 years or older may apply)

• CURRENT ELECTRIC BILL (complete bill, all pages)

Must cover at least 22 service days with meter reading.

• CURRENT GAS BILL (complete bill, all pages)

Must cover at least 22 service days with meter reading.

• PROOF OF CURRENT MONTHLY HOUSEHOLD INCOME

Income for everyone in the household. Must cover the last 30 days.

• CURRENT RENT RECEIPT, LEASE, SEC8 AGREEMENT, OR MORTGAGE STATEMENT

*ADDITIONAL DOCUMENTS MAY BE REQUESTED
