Independence at Home IAH offers no-cost programs and services designed to assist older adults and their caregivers.

Independence at Home is a community service of SCAN, a senior-focused organization founded in Long Beach, California in 1977. The goal then was to create a program to provide seniors easy access to community-based services that would allow them to remain healthy and independent at home. Today, Independence at Home offers a range of programs to meet the needs of seniors, disabled adults and their c

We are proud of participants who recently completed the Chronic Disease Self-Management Program (CDSMP), a six-week evidence-based workshop facilitated by Independence at Home.

Licensed through the Self-Management Resource Center, the program equips older adults and caregivers with practical tools to manage chronic conditions, set achievable goals, and navigate everyday health decisions with greater confidence.

A key benefit of our Multipurpose Senior Services Program (MSSP) is personalized Care Management.

MSSP Care Managers help eligible older adults (60+) stay safe and independent at home by providing comprehensive assessments, care planning, and coordination of services tailored to their unique needs.
