REQUEST FOR PROPOSAL
Project - Blacktop Sealing of Parking Lots for Fire Stations #1 & 2
Fire Station #1 – 2005 Lake View Road (including Firemen’s Club adjacent to fire station)
Fire Station #2 – 6100 Lake Shore Road
This project consists of repairing all cracks greater than 1/4 of an inch by machine cleaning and fill flush to the surrounding surface with NYS approved hot liquid asphalt.
Each contractor must contact the Project Manager, Roger Black, to schedule a walk through of each parking lot to confirm areas requiring repair.
Seal Coating:
1. Machine clean parking area and water flush if necessary.
2. All grease and oil deposits will be cleaned and petro sealed prior to seal coating.
3. Machine apply (non-spray) the first coat of NYS approved, coal tar base pavement sealer with two (2) to four (4) pounds of silica sand per gallon of sealer applied.
4. Brush (or machine) apply (no spraying) the second coat of sealer minus the sand.
5. Reapply all stripping for parking lanes including handicapped area only at Fire Station #1 and fire lane markings. NO handicapped markings are required for Station #2.
6. Contractor will be responsible to maintain traffic control in accordance with our schedule.
MATERIALS AND WORKMANSHIP SHOULD BE GUARENTEED FOR ONE FULL YEAR.
RFP Documents:
1. Proposals shall include furnishing all labor, materials, equipment and performing the Work for the above described project in strict accordance with the Proposal Documents and any Addenda, entitled as above.
2. Any exceptions to this proposal specifications shall be listed on a separate piece of paper attached to your quote. The exceptions should be noted and explained in detail and listing additional cost if any.

Contractors Representation:
In order to induce the Board of Fire Commissioners to accept a proposal, in addition to and not in lieu of any other representations and warranties contained in the
Proposal Documents, the Contractor represents and warrants the following to the Board of Fire Commissioners:
1. The Contractor is able to provide the plant, tools, materials, supplies, equipment, and labor required to complete the work and perform the Contractors obligations hereunder;
2. The Contractor is now and will continue to be authorized to do business in the State of New York. and is now and will continue to be properly licensed by all necessary governmental and public and quasi-public authorities having jurisdiction over the Contractor and over the Work and the Project;
3. The Proposal and ex*****on of the proposal documents and the Contractor's performance there under are within the Contractors duly authorized powers;
4. The Contractor has made an exhaustive study of the proposal documents; understands the terms and provisions thereof-and has sought or will timely seek any and all necessary clarifications prior to submitting the Proposal; and that the Proposal is made in accordance with the foregoing;
5. The Contractor has visited the project site and is completely familiar with the local and special conditions under which the work is to be performed and has correlated such knowledge with the requirements of the proposal documents;
6. The proposal is based upon the materials, systems and equipment described in the Request for Proposal documents without exception.
7. The Contractor certifies that his Proposal is submitted without collusion, fraud, or misrepresentation as to other Contractors, so that all Proposals for the project result from a free, open and competitive proposal environment.
8. The Contractor possesses a high level of experience and expertise in the business administration, management, and superintendence of projects of the size, complexity, and nature of this particular project, and that the Contractor will perform the work with care, skill and diligence of such a Contractor;
9. The Contractor acknowledges that the Owner is relying upon this Contractor's skill and experience in connection with the Work being proposal herein;
10. That complete sets of Proposal Documents were used in preparing the Proposal and that neither the Owner nor the Architect is responsible for errors or misinterpretations.
11. All proposals are to be received by the Secretary of the Lake View Fire District no later than 6:45 p.m. on Tuesday. July 7, 2026. Proposals can be submitted in person at that time, or by U.S. Postal Service mail to: Parking Lot Sealing; Lake View Fire District, PO Box 345, Lake View, NY 14085-0345. All proposals become the property of the Lake View Fire District and the Board of Fire Commissioners has the right to reject all proposals or accept any proposal that is determined to be in the best interest of the Lake View Fire District.
The foregoing warranties are in addition to, and not in lieu of (A) any and all other liability imposed upon the Contractor by law with respect to the Contractor's duties, obligations and performance of the Work and, (B) any and all other warranties, representations and certifications made in the Proposal Documents. The Contractor's liability hereunder shall survive the Owner's formal acceptance of and payment for the Work. All representations and warranties set forth herein and in the Contract Documents shall survive the final completion of the work or the earlier termination of this Agreement.
