We could not let Nurses Week end without shining the spotlight on the incredible nurses of Thome PACE! ๐
We proudly celebrate the compassionate, skilled, and dedicated nurses who care for our participants every single day โ in the clinic, in participantsโ homes, in the community, and behind the scenes coordinating the complex care that makes PACE so special.
Our clinic nurses coordinate care plans, manage medications, communicate with providers, respond to urgent needs, support transitions of care, and advocate tirelessly for each participantโs physical, emotional, and social well-being. Our home care nurses travel near and far to meet participants where they are, helping them remain safe, independent, and comfortable in their own homes while building meaningful relationships with participants and families alike.
We also want to recognize the nurses serving in essential non-clinical roles that are critical to the success of our organization. Our quality and compliance team members help ensure we continue to deliver safe, high-quality care while meeting important standards and improving outcomes for those we serve. Our Intake RN helps guide prospective participants and families through the enrollment process with compassion, knowledge, and support. Our Participant Care Manager works tirelessly behind the scenes to ensure every participantโs needs are addressed, services are coordinated, and care plans are carried out with excellence and dignity.
In the PACE model, nursing goes far beyond medical care โ it is compassion, teamwork, advocacy, education, coordination, and problem-solving all rolled into one.
Thank you for the countless phone calls, home visits, assessments, documentation reviews, care planning meetings, comforting conversations, and moments of kindness that often happen quietly behind the scenes. Your dedication helps our participants live with dignity, independence, and the highest quality of life possible.
