Myles is our Street Medicine van, and he helps our team take healthcare beyond our clinic walls and directly into the community.

For people experiencing unsheltered homelessness, getting to a clinic isn’t always possible. So our Street Medicine team meets people where they are, providing essential healthcare, building trust and helping patients connect to continued care and support.

And they don’t do it alone. Alongside partners across Houston’s homeless response system, our team brings together the people, resources and care needed to reach our neighbors where they are.

Because a health center is more than four walls. It’s the people, partnerships, and care that extend beyond them.

It’s National Health Center Week!
And we’re making it last all month long.

This week, we’re celebrating the people, partnerships + purpose that make our work possible and the belief at the heart of it all: everyone deserves access to quality, compassionate healthcare, regardless of housing status or ability to pay.

But we don’t just want to celebrate from our feed.
We want to celebrate with you.

The Health + Housing Summit offers a full day of learning, collaboration, and conversation across healthcare, housing, public health, and community services.

How can we continue building a healthier, more connected Houston together? One conversation, one encounter, and one act of care at a time.

Healthcare doesn’t begin at the clinic door. Through our Street Medicine program, we bring care directly to individuals experiencing homelessness across Houston, meeting people where they are with compassion, consistency, and dignity.

From wound care and medication support to behavioral health connections and pathways into ongoing treatment, every encounter is an opportunity to build trust, improve health outcomes, and strengthen connection to care.
