What you need for your appointment
Yourself - Any person requiring notarization of a document must appear in person in front of the notary. Identification - Any person requiring notarization of a document must produce identification to the Notary. The ID must be current, or if expired, issued within the last 5 years. It must have a photo of the document signer, a serial or another identifying number, a physical description, and signature. The name on the ID must match the name on the document being signed. Credible Witness - If you do not have an acceptable ID then you must bring two (2) people with you that can attest to your identity. They will need to provide their own identification and must give an oath/affirmation to your identity and place a thumbprint & sign the Notarial Journal - there is an additional $15.00 per witness cost for this. Acceptable forms of Identification:
