The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Rural Regional Reorientation (RRR) Program.

Applications are due by 11:59 p.m. CT on June 12, 2026.
RRR will provide grants of up to $4 million to eligible clinical and social service providers, either individually or in partnership with one or more entities.

The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Chronic Disease Management Program.

Applications are due by 11:59 p.m. CT on June 8, 2026.
The Chronic Disease Management Program is intended to fund providers, community-based organizations, and other organizations to implement and/or expand proven models for chronic disease management across rural regions of Oklahoma.

WIC participants may qualify for ride reimbursement to help with transportation costs to WIC appointments or grocery store trips.

You may qualify for:

A minimum of $20 per ride for up to two grocery store trips each month to your nearest grocery store

A minimum of $20 per ride for any WIC appointments

For longer trips, you’ll receive whichever is higher: the $20 per-ride minimum or $0.70 per mile

Many programs offer free or reimbursed rides to WIC appointments and grocery stores. Don’t let cost stop you—reach out today to see what’s available in your area!
