The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Chronic Disease Management Program.

Applications are due by 11:59 p.m. CT on Monday, June 8, 2026.

The Chronic Disease Management Program is intended to fund providers, community-based organizations, and other organizations to implement and/or expand proven models for chronic disease management across rural regions of Oklahoma.

Have you ever heard of Children First?

Children First is a free program for first-time mothers in Oklahoma that provides personal visits with specially trained nurses beginning early in the pregnancy and continuing through the child’s second birthday.

To be eligible, participants must meet the following requirements:
▪️ First-time mother.
▪️ Monthly household income at or below 185% of the federal poverty level.
▪️ Less than 29 weeks pregnant at enrollment.
