At Harriott Home Health Services, we are focused on connecting with patients no matter where they are. We care for patients all across Connecticut, from urban to rural communities. As part of our efforts to provide healthcare education and resources for our most vulnerable residents, we are proud to partner with Agape House for an upcoming community conversation next Wednesday, May 15th, at 12:00pm.

The Agape House Inc. of Bristol Ct. is a Christian nonprofit organization which serves the homeless and people in need in the Bristol community through multiple churches and multiple ministries. If you or your loved one have had to face this journey and struggled to find support and resources when it came to recovery and care at home, we invite you to join us at 26 Cross Street in Meriden this Saturday at 1:00pm. We'll be discussing patient rights and going over the resources that are available to you.

For the past four years, we've been proud to partner with Enfield Loaves and Fishes, Inc. to provide community education sessions for our most vulnerable residents. We provide information and resources regarding healthcare and other critical services.

Enfield Loaves & Fishes is a local area soup kitchen providing hospitality to all those in need by offering meals, aid, and companionship at no charge. The goal is to provide nutritious food, aid, and comfort and make a difference in the lives of individuals who live with food insecurity.

Our goal is to empower communities, patients, and their families with the knowledge they need to navigate the hospital discharge process and access the resources they need to ensure better health outcomes. This will be an inclusive space where you can build connections, gain insights, and learn more about vital resources.

Kamora’s Cultural Corner is an organization dedicated to supporting the LGBTQIA community by creating brave spaces to build and support organic community connections. You can learn more at kamorasculturalcorner.com.

Understanding your rights after being discharged from the hospital is essential. It is important to be informed about your options and to know that there is assistance available. Be sure to ask questions, take notes, and ensure that you understand all that is involved in your post-discharge care. Ask the staff to show you and your caregiver any other tasks that require special skills (like changing a bandage or giving a shot). Then, show them you can do these tasks. It's your right to be educated about you
