The Heart Matters Foster Family Agency

The Heart Matters Foster Family Agency The Heart Matters is dedicated to serving foster youth of the Inland Empire by providing quality foster care, mental health, and transitional housing services.

We care for children between the ages of 0-21 throughout San Bernardino & Riverside Counties. Foster Care is a service provided to ensure that children who must be separated from their families are cared for by nurturing families who are trained, licensed, certified, and approved to meet the child’s needs. The Heart Matters is involved in certifying all our parents. We not only conduct their first

interview but provide all training in-house including First Aid/CPR. Our staff also conducts all assessments and housing inspections to ensure our parents are ready emotionally and physically to foster children. Being involved in this process allows our agency to know our parents on a deeper level and match them with children who require care. Foster Care has a primary goal of providing opportunities for children and families to heal, grow, and develop so that children can be reunited with families who are able to provide safe, nurturing relationships intended to last a lifetime. The Heart Matters is currently recruiting parents and families who are willing to open their hearts and homes to children in need. If you are interested, please call (909) 906-1023 to start the process today! The Heart Matters Mental Health Center provides services to all foster youth within our agency. Services include, but are not limited to, traditional therapy, equine assisted therapy, play therapy, music therapy, and recreational therapy. Our highly trained clinicians provide youth ages 0 to 20 years old, with immersive mental health treatment to assist with their healing from past trauma and mental health challenges. Therapy is provided onsite, and when circumstances permit, within the community or in the home. Services are tailored to meet the needs of the youth through individual, family, and group settings. Our mental health staff and services are sensitive to all cultural values, sexual orientations, and gender identities, and are available in English and Spanish. The Heart Matters Mental Health Center is not currently accepting patients from outside of our agency. The Heart Matters Transitional Housing Program provides guidance and care to current and former foster youth between the ages of 18-21. Youth who are involved in the state foster care system struggle with basic common life skills that most take for granted. When these children turn 18, they enter the community with little to no support. Our Youth Service staff works to ensure that the young adults within this program become productive adult citizens that are educated, employed, and connected to their community. When a young adult is placed in our program, each individual is provided a fully furnished apartment that is preferably located near their current place of employment or school. Each apartment is provided for the youth rent-free along with their utilities and stipends for groceries. Our program also assigns each participant an Evolution Coach, which work tirelessly to make sure that the youth receive the support necessary for them to live successfully on their own. Our staff meets with each young adult weekly and is available 24 hours a day for crisis support. The Heart Matters believes that the extra support these exceptional young adults receive is what separates our program from others and leads to successful outcomes. If you, or someone you know, is in need of Transitional Housing services for foster youth ages 18-20, please contact us at (909) 906-1023.

The graduation baskets are officially heading out this week 🎓💛

Each basket was thoughtfully put together to celebrate our incredible graduates and the huge milestone they’ve achieved. Filled with a Stanley cup, customized blanket, gift cards, heartfelt notes from our staff, and the classic Oh, the Places You’ll Go! book, we wanted every youth to feel celebrated, supported, and excited for what comes next.

A very special thank you to Assistance League of Riverside for helping make this possible. Your generosity and support truly helped us create something meaningful for our youth as they step into their next chapter.

We are SO PROUD of our graduates and can’t wait to continue celebrating our youth through the many initiatives still ahead this year.

If you would like to get involved, partner with us, or support future initiatives, please reach out!

“Oh, the places you’ll go.” 🎓💛

For youth in foster care, graduation is so much more than a ceremony. It represents resilience, determination, and overcoming challenges many could never imagine.

Nationally, less than 60% of foster youth graduate high school by age 19, making this accomplishment something truly worth celebrating.

This year, we are honored to gift each of our graduates a copy of Oh, the Places You’ll Go! In their graduation baskets as they step into their next chapter filled with hope, growth, and opportunity.

We are so incredibly proud of our youth and all they have accomplished.

If you would like to help us celebrate and support our graduates, please DM us or email us at mailto:[email protected] 💙

Some youth need more than a place to stay—they need someone willing to see their resilience, believe in their potential, and walk alongside them as they heal.

Our Enhanced Intensive Services Foster Care (EISFC) program supports youth with the highest level of need through specially trained resource parents and dedicated team that provides ongoing guidance, specialized services, and one-on-one support every step of the way.

The program is designed to ensure both youth and resource parents have the tools, stability, and partnership needed to succeed. You don’t have to do this alone. We will be with you every step of the way. If you’ve ever wondered whether you could make a difference in a young person’s life, we would love to talk with you.

Mental Health Matters. Every day, not just this month.

Taking time to care for your mind, staying connected, and finding moments to slow down can make a real difference. Whether it’s a walk, a conversation, or simply taking a breath, small steps matter.

This month is a reminder to check in with yourself and those around you. You are not alone.

May is Foster Care Awareness Month 💙
It’s a time to uplift the voices of youth, recognize the impact of those who step in to care, and reflect on how we can all play a role in supporting our community. From resource parents to partners, every act of support helps create stability, connection, and opportunity.

If you’ve ever thought about getting involved, this is your sign. It takes a village, and there’s a place for you in it.

We’re so proud to celebrate 8 incredible graduates this year 🎓❤️
For many youth in foster care, graduation isn’t guaranteed—less than 60% graduate high school. That’s what makes this moment even more meaningful.
Each of these young people has worked hard to stay on their path, and we couldn’t do it without our community.

We need your help! To sponsor a graduate and contribute to a special gift basket—with a personal message included—please reach out, we would love to have you be part of this moment.

Moments like these don’t happen without community. 🐴🤎

Experiences like equestrian give our youth the chance to try something new, build confidence, and simply enjoy being kids. These opportunities matter—and they’re made possible by partners who choose to show up and invest in our youth in meaningful ways.

We’re so grateful for the impact our community continues to make.

Not everyone hears the call—but if you do, it matters. ❤️

There are youth in our community who don’t need perfect… they need presence, consistency, and someone willing to show up.

If this has ever crossed your mind, it’s worth exploring. We’re here to walk with you every step of the way.
