We care for children between the ages of 0-21 throughout San Bernardino & Riverside Counties. Foster Care is a service provided to ensure that children who must be separated from their families are cared for by nurturing families who are trained, licensed, certified, and approved to meet the child’s needs. The Heart Matters is involved in certifying all our parents. We not only conduct their first
interview but provide all training in-house including First Aid/CPR. Our staff also conducts all assessments and housing inspections to ensure our parents are ready emotionally and physically to foster children. Being involved in this process allows our agency to know our parents on a deeper level and match them with children who require care. Foster Care has a primary goal of providing opportunities for children and families to heal, grow, and develop so that children can be reunited with families who are able to provide safe, nurturing relationships intended to last a lifetime. Services include, but are not limited to, traditional therapy, equine assisted therapy, play therapy, music therapy, and recreational therapy. Our highly trained clinicians provide youth ages 0 to 20 years old, with immersive mental health treatment to assist with their healing from past trauma and mental health challenges. Therapy is provided onsite, and when circumstances permit, within the community or in the home. Services are tailored to meet the needs of the youth through individual, family, and group settings. Our mental health staff and services are sensitive to all cultural values, sexual orientations, and gender identities, and are available in English and Spanish. The Heart Matters Mental Health Center is not currently accepting patients from outside of our agency. The Heart Matters Transitional Housing Program provides guidance and care to current and former foster youth between the ages of 18-21. Youth who are involved in the state foster care system struggle with basic common life skills that most take for granted. When these children turn 18, they enter the community with little to no support. Our Youth Service staff works to ensure that the young adults within this program become productive adult citizens that are educated, employed, and connected to their community. When a young adult is placed in our program, each individual is provided a fully furnished apartment that is preferably located near their current place of employment or school. Each apartment is provided for the youth rent-free along with their utilities and stipends for groceries. Our program also assigns each participant an Evolution Coach, which work tirelessly to make sure that the youth receive the support necessary for them to live successfully on their own. Our staff meets with each young adult weekly and is available 24 hours a day for crisis support. The Heart Matters believes that the extra support these exceptional young adults receive is what separates our program from others and leads to successful outcomes.
