Youth and young adult homelessness exists in our community.

The Community Housing Navigation Program (CHNP) assists unaccompanied youth and young adults, 24 years or younger, who are experiencing homelessness, at risk of experiencing homelessness and fleeing from domestic violence find permanent, safe places to lay their heads at night.

Since its inception in 2022, the CHNP program has assisted 63 young adults in Region 4 (Grand Forks, Walsh, Nelson & Pembina Counties).

Between July 1st, 2025 - March 30, 2026, in Grand Forks County alone, 140 elementary through high school students in the school district experienced housing insecurity.

What Does a Permanent Supportive Housing Case Manager Do?

We do more than help people find housing—we help them keep it and truly feel at home. As Permanent Supportive Housing Case Managers, we walk alongside individuals who have experienced homelessness, mental health challenges, or other barriers, providing ongoing support, resources, and encouragement.
From connecting clients to medical and mental health services, to helping build life skills, navigate income, and create stability—we’re there every step of the way. Our goal is simple but powerful: safe housing, lasting stability, and a better quality of life.

Because housing isn’t just a place—it’s the foundation for everything else. 💙
