Mental Health Connections

Mental Health Connections Connections offers answers, options, and support to individuals and families. It also works closely with schools through a school liaison.

Connections is a program of the Health District of Northern Larimer County in partnership with SummitStone Health Partners. Services include needs assessment, information and referral, assistance with coordinating and navigating care, brief intervention, connection to reduced-cost counseling, education and training. CAYAC (Child, Adolescent, and Young Adult Connections) is a service of Connections

that helps young people (up to age 24) and their families with the early identification, assessment/testing, and treatment of mental health and substance use concerns. The Health District of Northern Larimer County maintains this page to inform the community about the services and activities of Connections. We reserve the right to remove comments/materials from Health District social media tools when, in the Health District’s sole discretion, those comments/materials:

• Are potentially libelous;
• Contain obscene or sexually explicit content or language;
• Are hateful or mean-spirited;
• Foster or perpetuate discrimination on the basis of race, creed, sexual orientation, age, religion, gender or national origin;
• Contain personal attacks, insults, profanity, name-calling, or threatening language;
• Violate a legal ownership interest, such as copyright or trademark, of any party or contain plagiarized material;
• Publish private, personal information without consent;
• Solicit commerce, including but not limited to advertising of any business or product for sale;
• Are off topic or link to material that is off topic;
• Contain embedded images from external sources;
• Violate any law or promote the violation of any law;
• Encourage or constitute prohibited discriminatory or harassing conduct;
• Are made by a person claiming to be someone else; or
• Support or oppose any political campaign or ballot measure. Additionally, the Health District reserves the right to terminate a person's ability to post comments/materials to or otherwise participate in Health District social media tools when that person has repeatedly posted any of the above listed inappropriate comments/materials.
