NASEMSO Consolidated Blood and Blood Product Guidance - Now Available

Overwhelming evidence over the last few decades has demonstrated the potential lifesaving capacity of timely prehospital transfusions of whole blood and/or blood products in the treatment of hemorrhagic shock. Dating back to the 1966 release of Accidental Death and Disability: The Neglected Disease of Modern Society emergency medical services (EMS) systems have been working to save the lives of those in traumatic incidents. As of a March 2025 estimate, 2% of EMS agencies have prehospital blood programs and an estimated one million patients could benefit from its use annually. Research illustrates the lifesaving capacity of prehospital blood transfusions, but no state level evidence-based guidance exists to promote widespread implementation.

To fill this gap and in light of the rapidly evolving spectrum of whole blood and blood products in the prehospital management of hemorrhaging patients, NASEMSO has developed two distinct documents to help guide state EMS systems, policy makers, and local clinical EMS providers as they develop whole blood and blood product programs in their communities.

Prehospital Transfusion of Whole Blood and Blood Products: Considerations for State Officials provides overarching guidance for policy considerations related to program regulation. It is a multi-disicplinary approach with input from all NASEMSO Councils and Committees.

NASEMSO Blood Product Administration Documentation V.2 provides specific guidance for patient care reporting and National EMS Information System (NEMSIS) data collection.

Both documents are publicly available for download on our website under: Resources/Documents/Policy and Guidance Resources/Prehospital Blood

We anticipate updating these documents as well as developing new guidance as the need arises.

Thank you.

Did you know, that data from the National Highway Safety Administration (NHTSA) indicates that 43% of the people who died from injuries received during a motor vehicle crash were still alive when emergency services arrived on scene? This is why post-crash care and the systems which provide it are vital in supporting the National Roadway Safety Strategy. NASEMSO is committed to improving post-crash care and prehospital blood and blood product administration is one of the most promising new developments for saving lives.
