Recognizing Excellence in Patient Safety and Ready Reliable Care

Please join us in recognizing recent Defense Health Agency award recipients whose actions reflect the culture of safety, accountability, and high reliability that define the 60th Medical Group.

The awards were presented by Col. Patrick Parsons, commander, 60th Medical Group, in recognition of both team and individual achievements that exemplify Ready Reliable Care in action.

The nurses and technicians of the Emergency Department, Intensive Care Unit, Medical-Surgical Unit, and Laboratory were recognized through the Defense Health Agency Ready Reliable Care Reliability in Action program for exemplifying the principle of Sensitivity to Operations through collaborative efforts to reduce blood culture contamination and improve patient safety outcomes across David Grant Medical Center. Through standardized device use, cross-section training, and data-driven process improvement, the team reduced contamination rates to a low of 0.81%, helping reduce unnecessary treatments, costs, and extended hospital stays while strengthening patient care.

A1C Juana Conde was also recognized as a Defense Health Agency Patient Safety Champion for her dedication to patient advocacy and commitment to doing the right thing when it mattered most. Her willingness to think critically, speak up, and coordinate emergency transport for a high-risk patient helped prevent further adverse outcomes and reflected the sound judgment and accountability that strengthen a culture of safety.

Whether through team-based process improvement or individual acts of patient advocacy, these recognitions highlight the shared commitment of our Golden Bear Medics to delivering safe, reliable care every day for our patients and beneficiaries.

Recognizing Excellence in Patient Safety

Please join us in recognizing A1C Kriziah Juaban, one of the youngest recipients within the 60th Medical Group to receive this Defense Health Agency Patient Safety Champion recognition, for her vigilance, professionalism, and commitment to patient care.

While reviewing a prescribed treatment plan, A1C Juaban identified a critical contraindication that could have resulted in patient harm. She immediately elevated her concerns through the appropriate channels and continued advocating for patient safety even after the initial plan was revised, ultimately prompting further provider reassessment and additional safeguards in the patient’s care.

Her actions reflected moral courage, sound clinical judgment, and the type of high reliability mindset that strengthens a culture of safety across David Grant Medical Center every day.

The recognition was presented by Col. Patrick Parsons, Commander, 60th Medical Group.

A1C Juaban’s example reinforces an important message throughout military medicine: every member of the healthcare team, regardless of specialty, rank, or experience level, plays an important role in fostering a culture of safety, accountability, and Ready Reliable Care for our patients and beneficiaries.
