~How AFHs operate
~What you or your loved one’s day-to-day life may look like
~How funding works
~What to watch for (good and bad)
~How to decide if it’s the right fit
This will be a discussion-based session, not a lecture, so bring your specific questions.

CARE COORDINATION SURVEY

WA INCLUDE is excited to launch a new survey on Care Coordination!

Dr. Thanh P. Nguyen, PhD. from WA INCLUDE needs your input on care coordination for autistic youth with mental health needs. This needs assessment survey asks about your experience with care coordination---service that helps youth and families find, connect, and manage health, school, and community supports.

Eligibility: To participate, you must be 18+ years or older to participate and have received or provided care coordination services for autistic youth with mental health needs.

Typically this includes: families, caregivers, autistic individuals, and service professionals (coordinators, family navigators, case managers, etc.)

Compensation: Participants may enter a random drawing for a $30 e-gift card.

Privacy and Confidentiality: Your participation is anonymous and voluntary. The survey does not collect names or identifying information. Results will be reported in group form only, and employers or service agencies will not know if you participated.
