Our mission is to end the cycle of homelessness by providing quality care in a safe and comfortable environment for those who need further support. Horizon is proud to offer a range of services. We ensure residents are housed, recover from post-acute care, take medication, and attend all follow

-up appointments. At the same time, we help residents obtain long-term housing and set them up for success.

A big thank you to East Valley Community Health Center for providing primary care services to our clients, who we've had such a hard time connecting with appropriate providers. Today, they started with 10 clients. They provided services such as physicals, bloodwork, medication recommendations/ refills, and specialty referrals. We are grateful for the opportunity to coordinate this event. Our clients were happy, engaged, and appreciative of the care they received.

This is one of our recent discharges Victor E. Through our company, he was able to reconnect with his family that has been searching for him for years and had no idea that he was homeless. He was able to reconnect with his aunt, and he has now been discharged and reunited, living with his uncle. His Aunt is in the picture, and she expressed her gratitude for our help in keeping him safe. They are glad to have him back with the family.
