THE MISSION: Protect and promote personal, family and community health; ensure a safe and healthy environment; and prevent or control the incidence and spread of disease. The Surry County Health and Nutrition Center provides the following services:

CHILD HEALTH SERVICES: Involves all aspects of pediatric assessment and health care for patients to 18 years of age and for those patients with Medica

id to 21 years of age. SENIOR SERVICES: This division provides in-home care to elderly and disabled individuals wishing to remain at home, because, truly, "there's no place like home..." FAMILY CARE COORDINATION SERVICES: This program provides case management for high-risk pregnant women and children up to 5 years of age. NUTRITION/WIC/DIABETES: These services include the Supplemental Nutrition Program for Women, Infants, and Children (WIC), Breastfeeding Promotion and Support, Community Nutrition Education, Medical Nutrition Therapy, and Diabetes Self-Management Education. Our nutrition services are designed to promote healthy lifestyles for people of all ages. HEALTH PROMOTION/HEALTH EDUCATION: This section provides health promotion and disease prevention services at schools, worksites, and the community. COMMUNITY HEALTH SERVICES: These services include Immunizations, Adult Primary Care, Family Planning, STD/Communicable Disease, and other services for the protection of the community's overall health. ENVIRONMENTAL HEALTH: The principal function of this section is to prevent disease by ensuring that no unsafe food or water is consumed. Men are often less likely to visit the doctor or talk about health concerns. Regular checkups can help find problems early, when treatment works best. Simple steps like eating healthy foods, staying active, getting enough sleep, and managing stress can improve overall health. SCHNC encourages men to take time for their health this month and all year long.

What’s something you need but can’t find, or something that’s just too hard to access around here?

Healthcare? Food? Transportation? Support for your family?

We’re inviting residents to a Community Input Session to tell us about real needs and real barriers. No speeches, no pressure. Just a chance to be heard.

We want to hear from you about:

> The main health issues and concerns in your community

> Barriers that make it harder to improve your health

> Resources that are available, and resources that are still needed

> Possible solutions and ways to bring about change

Your experiences are more than stories; they're lessons we can learn from. By taking part, you help guide local priorities, inform future action, and make sure community voices are part of the decisions that follow.

What is a Stroke?

A stroke happens when blood flow to part of the brain is blocked or reduced. When this happens, brain cells begin to die within minutes. Stroke is a leading cause of disability and death in the United States. Quick treatment can save lives and reduce long-term damage.
