5th Grade Completion Ceremony Information

Thank you to those who have already completed pre-registration for your child’s 5th Grade Completion Ceremony!

Ceremony Schedule:

Tuesday, May 26th @ 9:15am
Stultz, Hann, Dewise, Maddox, & Bowden

Wednesday, May 27th @ 9:15am
O’Rourke, Hile, Wittenberg, & Bracewell

Our office has prepared visitor badges for guests whose information is already on file in our school security system.

If your information was not found, it will need to be entered manually upon arrival.

**Important Reminder:**
All visitors **must bring a valid ID** to receive a visitor badge. Please arrive early to allow time for check-in.

Congrats to each of our 8 students who became official members of the Antioch MILLIONAIRES CLUB!
Each of these students read more than 1,000,000 words this year through our Accelerated Reader program!
We celebrated this tremendous accomplishment today with medals, lunch at Bamboo, and the Destin Party Bus! Congratulations to these amazing readers!

Antioch Clinic medication pick up

Mrs. Samantha in the clinic would like to remind families that all medications signed into the clinic must be picked up by 1:00pm on Thursday, May 28, the last day of school. Any medication not picked up by 1:00pm will be promptly disposed of in accordance with school policy. Thank you for your attention to this important reminder.

KG and new student registration in Okaloosa County is open through the FOCUS system.
Parents are required to initiate the enrollment process by creating an account in the FOCUS Parent Portal.

Please note that FOCUS is address-specific for the school your address is zoned for. Make sure you are selecting the 2026/27 Boundaries on the school site locator webpage.

Once you have completed the partial online registration, contact Antioch Elementary to set up an appointment for you to come in and finish your child’s registration.

During this appointment, you are required to bring the following documents to your appointment:
1. Two Proof of Residency:
ONE of the following is REQUIRED: a lease OR a mortgage document covering the current year.
The second item can be:
•Parent/legal guardian’s driver’s license, FL ID card, or Military ID with the registering address
•Electric bill/water bill
•Homeowner's insurance policy or bill for registering address dated within the last 30 days
•Mail from employment and /or financial institutions, including checking, savings, property tax records, credit card statements, or investment account statements, dated within the last 30 days
•Mail from Federal, State, County, or City government agencies to the registering address, dated within the last 30 days

2. Child's Birth Certificate
3. ENROLLING Parent/Guardian driver’s license
4. Current school physical – dated 1 year before the start of this school year.
5. Florida immunization form 680 – can be obtained either from the Florida Health Department or, if you are military, from the Eglin Immunization Clinic)
6. Any paperwork from the previous school
7. Any legal documents regarding custody, if applicable

Please note that kindergarten students must have their Florida Immunization Form 680 and a current school physical (dated no later than twelve months before school starts) turned in before the start of school.

We encourage families to complete enrollment as soon as possible, as classes will be formed, teachers assigned, and materials ordered based on student enrollment.
