Are you changing doctors? Do you need your immunization records for a new school? Getting copies of your medical records can be a real challenge if you’ve seen different providers throughout your life. If you can’t easily access your medical records online, you’ll likely have to request the records from your medical provider in writing. Each one may need you to follow a different process when asking for your record.
Explore these tips on how to request your health record and know your rights.
Check your doctor's website. If you regularly visit a doctor's office, they may have details on how to request your records on their website.
Call or e-mail your doctor. By doing so, you can ask how you can request your health record.
Fill out a health or medical release form. Many doctors have a form you can fill out either online or on paper to access your information.
Pay any fees. You may have to pay for the cost of your medical records if you want them delivered on paper, by fax or electronically.
Wait up to 30 days. If your provider needs more than 30 days, they must give you a reason for the delay and a new delivery date.
