Thousand Islands Emergency Rescue Service is the 9-1-1 ambulance service provider Paramedic Level Service to the residents and visitors of the Town of Clayton and the Town of Orleans. Our response district is 176 square miles of mostly rural terrain and encompasses a large portion of the St. Lawrence River along the US/Canada border. We staff paramedics 24/7/365 and supplement our capabilities wit

TIERS was dispatched this morning to a report of an injured person on Murray Isle. A question we often hear is: "How does TIERS respond to calls on the water?"

As it turns out, our ambulances don't float well—Well, we haven't actually tested that theory, but we've agreed its a safe assumption.

When emergencies occur on the mighty St. Lawrence River, TIERS is dispatched simultaneously with the Town of Clayton Fire Department. Together, we respond aboard their fireboat, The Last Chance.

Utilizing a boat captain and personnel from the fire department, along with an EMT and Paramedic from TIERS, residents of the islands and those recreating on the river have access to the same high level of prehospital emergency care available on the mainland should things go awry.

This collaboration is well-planned, regularly trained on, and thoroughly rehearsed, ensuring that when seconds count, both agencies can work seamlessly together to bring emergency medical care to even the most remote locations in our service area.

"The Last Chance" also serves as a Fire Engine on the water, capable of pumping over 2000 gallons of water per minute. No hydrant required.

Whether by road or by water, we're committed to providing timely, professional care whenever and wherever it's needed. Fortunately, the patient involved in this morning's incident sustained only minor injuries and is expected to make a full recovery.

An Inside Look at Our New Lifesaving Tech! 👀
When a patient is in critical condition and can't breathe on their own, our paramedics have to act fast. Thanks to the funds raised at last year's TIERS Annual Gala, that job just got a whole lot safer.

We have officially purchased 5 video laryngoscopes! These devices use a small camera to help paramedics visually guide a breathing tube into a patient's airway with precision.

This group of people, plus many more not pictured, are what make TIERS go round. This group is not just our members, but their families as well. We thank them all. Those that respond to the calls, and those who are understanding when their family members leave to help a stranger on a moments notice.

The EMS life is a weird one. We meet people on their worst days, and in their worst and saddest moments. It’s a reality of this calling, but what it also does is brings us closer. These people are a family. A dysfunctional one at best, but a family.

So today, in honor of EMS Week 2026, we had a family cookout. Some burgers, some brisket, some yard games, and a whole bunch of stories and laughs. Unfortunately, today’s on duty crew is not pictured here. In true EMS traditional fashion, the alarm went off just as the food was coming off the grill.
