I'm a licensed Day Care provider seeking children to love and nurture while your at work or school. licensed by the State. Monday thru Friday 6am to 6pm. I Also have evening hours as well. A note to parents: This form is meant to measure and document what a child knows and to track their progress if you do the assessment at different times of the year. Many children do not master all these skills before entering kindergarten and that is OK.
