Reach for the Stars Case Management We are a Case Management agency funded by the Supports for Community Living and Michelle P. Waiver.

We will work with you to:
--provide services and supports in a manner that is respectful and honors the choices of each person and his or her family and friends.
--to promote self-advocacy in order for each person to take their place as a full contributing member in their community.
--to effectively address the unique and challenging needs of each individual served through collaboration with fami
